Taiwan's medical bills for foreigners can be brutal. We compare AXA, MSIG, Allianz, Roojai, Muang Thai Life and Tipco side-by-side — with real scenarios and honest disclosures on what each actually covers.
Taiwan is safe — crime is low, infrastructure excellent, people friendly. But Taiwan's healthcare system is not designed for uninsured foreign tourists. The National Health Insurance (NHI) only covers residents with a valid NHI card. As a tourist, you pay full foreigner rates — and they are substantial.
For a serious emergency — a fracture, appendicitis, or cardiac event — a private hospital in Taipei may require a deposit of NT$10,000–50,000 (approximately THB 11,000–55,000) before beginning treatment. A 3-night hospital stay can easily run NT$150,000–300,000 (THB 165,000–330,000). Beyond medical costs, Taiwan sits squarely in typhoon season from June through October: Typhoon Bebinca in September 2024 cancelled hundreds of flights at short notice. The April 2024 Hualien earthquake disrupted Taroko Gorge access for months. These are not hypothetical risks.

AXA is one of the most recognised insurance brands in Thailand, and the SmartTraveller / SmartTravel Asia plans offer high medical limits with a genuine family option that covers the whole group under one policy. Strengths: High coverage ceiling, broad inclusions, Thai-language 24h hotline reachable via a local Bangkok number, smooth online purchase flow. Claims are generally processed in 14–30 days with Thai-language documents. Watch out for: Adventure activities such as trekking above a certain elevation, scooter rental (without local licence), and recreational scuba are typically excluded from the base plan — you will need to add a rider, which increases the premium.
MSIG Easy Travel is the most budget-competitive of the six — offering the lowest entry price while still providing respectable medical coverage. Strengths: Transparent pricing, easy online purchase via MSIG's website, Rabbit Care or Frank.co.th, fast-responding Thai 24h hotline, and a solid track record of straightforward claims settlement. Watch out for: The cheapest plan tier has a lower medical limit than we'd ideally recommend for Taiwan — upgrade to the Premium tier for a trip to Taipei. Adventure activity coverage is very limited across all tiers; scooter or scuba in Penghu is almost certainly excluded.
Allianz Travel Care offers the highest medical coverage ceiling of the six — up to THB 10M on the premium plan. The network of partner hospitals in Taiwan includes major private hospitals in Taipei, and the pre-authorisation (cashless treatment) system means you may not need to pay upfront at all if you are admitted to a network hospital. Strengths: Exceptionally high limits, strong global partner hospital network, relatively smooth pre-auth process when you call the hotline immediately upon hospital admission. Watch out for: Higher premium than MSIG or Roojai; pre-existing condition exclusions are strict — undisclosed conditions at point of sale can void claims.
Roojai is Thailand's leading digital-first insurer — everything is designed to be purchased, managed and claimed via the app or website, with no intermediary commission inflating the price. The result is consistently lower premiums than going through a traditional agent. Strengths: Excellent UI/UX — quotes, policy purchase and claims initiation all take under 5 minutes. Thai-language 24h line responds quickly according to Google and Pantip reviews. Transparent pricing with no hidden fees. Watch out for: As a newer brand, Roojai has fewer years of large-claim history than AXA or Allianz. Reviews are mostly positive but the sample size on Pantip is smaller than for established players.
Muang Thai Life Insurance stands out for its flexibility around age and health conditions — policies cover a broader age range and the underwriting is often more accommodating of pre-existing conditions than competitors. Strengths: Nationwide agent network across Thailand makes in-person consultation easy; fully Thai-language service from start to claim; MTL 24h hotline covers both general queries and emergency medical assistance. Watch out for: Purchasing online is not as smooth as Roojai or AXA — some plans require going through an agent, which adds friction. Adventure activity coverage is limited.
Tipco TravelGuard is underwritten through the AIG (American International Group) network — giving it access to one of the widest global hospital and assistance networks in the industry, including strong coverage in Taiwan. Strengths: AIG's international 24h emergency coordination is robust, hospital partnerships in Taipei give cashless treatment options at select facilities, and the plan is popular among frequent travellers on Pantip. Medical, trip cancellation and personal property coverage are all solid at the premium tier. Watch out for: Premium pricing above the mid-market rate; claims routing through the international AIG system can involve more documentation steps than a Thailand-only insurer.
