Take Keikyū to Shinagawa and switch to the JR Yamanote loop for Shibuya, Shinjuku, and Ueno, or ride the Monorail to Hamamatsuchō and join the Yamanote there. Staying in Asakusa? A Keikyū through-service runs there directly, no transfer needed.
For Tokyo Disney Resort, the direct limousine bus from the airport is easiest (~45–60 min). For Yokohama, ride Keikyū straight south to Yokohama Station in around 30 minutes. Neither route requires going through central Tokyo first.
For Kyoto, Osaka, or the north, head to Tokyo Station or Shinagawa (both have shinkansen). With a JR Pass, take the Monorail to Hamamatsuchō, transfer to the Yamanote for Tokyo Station free of charge, then board the shinkansen from there.
